The Purchasing Coordinator is responsible for receiving and processing all departmental requisitions and purchase orders, maintaining appropriate inventory levels in accordance with CHC procedures, and maintains control of supplies and equipment for all CHC departments. The Purchasing Coordinator provides support to the Director of Purchasing in creating and maintaining meaningful partnerships with vendors to assure the constant procurement of supplies. The Patient Educator works with patients referred for assistance by educating and coordinating services for chronic disease self-management. The purpose of the Patient Educator is to improve health outcomes through coordinating care, educating patients, building trust between patients and medical practitioners, enhancing communication and the continuity of care. The Patient Educator also work closely with the Quality Department to help close care gaps and improve health outcomes along with implementing changes relating to quality compliance. The Patient Educator will consult with other health care team members to coordinate services of patient educations, preventive care, and disease management.
